This position will oversee patient services across the state, manage at least two patient services coordinators, create and implement new programs, develop relationships in the Middle TN region with service providers and provide patient services to patients inthe Middle TN area. ALSTN 2 post s 15-Jul-2008 12:53 PM	The ALS Association TN Chapter seeks full time professional for Patient Services Coordinator in Memphis and West TN. Responsibilities include: Responding to patient and caregiver inquiries for information and referrals Conducting telephone consultation with patient and/or caregiver on a regular basis Providing semiannual in-home support to patients and families counseling patients and families on the progression of ALS, what to expect and how to prepare Documenting all patient contact and maintaining up to date records in the data base Developing partnerships and collaborations with service providers, hospitals and clinics that serve ALS patients’ needs Maintaining a collaborative relationship with medical equipment providers Maintaining and coordinating the West TN Equipment Loan Closet Planning and coordinating the annual West TN Symposium for patients, care givers and service providers Representing ALSA through in-service presentations Contributing ideas and articles for chapter newsletter Working in collaboration with other patient services coordinators across the state Assisting with outreach to patients and caregivers for fundraising purposes The ideal candidate will be a Licensed Clinical Social Worker, Occupational or Speech Therapist, an RN/ LPN or a person with case management or palliative care experience or comparable experience.
